Survival and Cardiovascular Outcomes of Patients With Secondary Mitral Regurgitation

Background: The literature on secondary mitral regurgitation (MR) has evolved significantly over the past decade, addressing the complexities of its diagnosis, management, and outcomes. The foundational work by Kibar Yared, Kaitlyn My-Tu Lam, and Judy Hung (Yared et al., 2009) highlights the multifactorial origins of MR, particularly emphasizing the role of left ventricular (LV) dysfunction in secondary MR. Literature Review: Treede and Charitos (Treede & I Charitos, 2019) build on this foundation by establishing the prevalence of secondary MR in heart failure (HF) patients as a significant prognostic factor. Their findings indicate that secondary MR is a predictor of mortality, highlighting the need for tailored treatment strategies that incorporate both surgical and interventional approaches. Lavall et al. (Lavall et al., 2021) further investigate intervention techniques, specifically transcatheter mitral valve annuloplasty, revealing a concerning mortality rate of approximately 50% at four years post-procedure. Nappi et al. (Nappi et al., 2021) advocate for a multidisciplinary approach to managing secondary MR, suggesting that while mitral valve repair has been traditionally favored, recent randomized controlled trials indicate that replacement may be more beneficial in severe cases. Pavon et al. (Giulia Pavon et al., 2022) expand on diagnostic capabilities, showcasing the role of cardiovascular magnetic resonance (CMR) in assessing LV dysfunction associated with secondary MR, which can inform treatment decisions, particularly regarding surgical revascularization and percutaneous interventions. Conclusion: In conclusion, the literature presents a comprehensive overview of the evolving landscape of secondary MR, emphasizing the critical interplay between diagnosis, management strategies, and patient outcomes. The collective insights from these studies underscore the importance of individualized treatment approaches, the need for further research into gender differences, and the potential of advanced diagnostic tools to enhance clinical decision-making in managing secondary MR.
