Abstract

Introduction: Despite high-intensity statin therapy, many patients with atherosclerotic cardiovascular disease (ASCVD) have persistently elevated LDL-cholesterol levels and remain at high risk. Proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors are potent lipid-lowering agents. This review assesses their impact on cardiovascular outcomes.

Methods: A search for cardiovascular outcome trials of PCSK9 inhibitors (alirocumab, evolocumab) was conducted. We included placebo-controlled RCTs in patients with ASCVD on maximally tolerated statin therapy. The primary outcome was a composite of MACE.

Results: The meta-analysis of three large-scale trials demonstrated that the addition of a PCSK9 inhibitor to statin therapy resulted in a significant 15% relative risk reduction in MACE. This was accompanied by an approximate 60% reduction in LDL-cholesterol levels.

Discussion: PCSK9 inhibitors provide significant additional cardiovascular risk reduction when added to statin therapy in high-risk patients. Their primary role is in secondary prevention for patients who cannot achieve LDL-C goals with conventional therapy.

Conclusion: PCSK9 inhibitors significantly reduce cardiovascular events in high-risk patients with ASCVD when added to maximally tolerated statin therapy.
