Introduction: Hawthorn extract is a well-known herbal remedy in Europe for cardiovascular conditions. Its potential as an antihypertensive agent is of increasing interest. This review evaluates the clinical evidence for the efficacy of hawthorn in managing hypertension.
Methods: We conducted a systematic search for RCTs comparing hawthorn extract to placebo or antihypertensive drugs in patients with hypertension. Changes in SBP and DBP were the primary outcomes.
Results: Eight RCTs were included in the review. The results were mixed, with some studies showing a modest but significant reduction in diastolic blood pressure, particularly in patients with co-existing diabetes. However, a consistent effect on systolic blood pressure was not observed across all studies.
Discussion: Hawthorn may offer a small benefit in lowering blood pressure, possibly through its vasodilatory and mild diuretic effects. The evidence is not robust enough to recommend it as a standalone treatment for hypertension.
Conclusion: The evidence supporting hawthorn for the treatment of hypertension is currently inconclusive.
